Medication preparation and discrepancies · NCLEX-RN Foundations

12 min + practice · Original foundation lesson

Interpret client cues and justify medication preparation and discrepancies within the current assessed situation.

Recognize what is known before choosing an action

This lesson develops medication preparation and discrepancies. Start with the actual client cue, not an assumed diagnosis. Distinguish a measured finding from an interpretation, and ask what the current situation requires. A previously stable assessment, a familiar routine or completion of an intervention does not prove that the current outcome is safe. Make the relevant uncertainty visible and use the authorized clinical process to resolve it.

Questionable medication sterility

A medication container with questionable sterility is not used. Follow the relevant pharmacy and infection-prevention procedure rather than trying to visually certify sterility. Worked case: A medication vial has been entered with a used syringe. A justified response is: Discard the compromised vial using the required procedure and obtain an appropriate uncontaminated medication supply. Contrast: Use the contents because the solution is still clear and no contamination can be seen through the vial. This alternative fails because appearance does not establish sterility.

Clean medication preparation

Safe medication preparation protects sterility and stability. Use an appropriate preparation area and aseptic process; do not invent shortcuts to compensate for an unsuitable environment. Worked case: Injection preparation is proposed beside a contaminated sink area. A justified response is: Use the designated clean medication preparation area with appropriate supplies and aseptic technique before drawing up the injection. Contrast: Prepare the injection near a known contamination source because the medication itself is expected to be effective. This alternative fails because medication preparation requires a suitable clean area.

Handling multidose vials

A multidose vial that enters an immediate patient-treatment area must be dedicated to that patient only; moving it back does not restore shared-use eligibility. Permitted shared vials stay in a designated clean preparation area, and no vial is re-entered with used injection equipment. Worked case: A multidose vial is proposed for shared use in a treatment bay. A justified response is: If the vial enters an immediate patient-treatment area, dedicate it to that patient; permitted shared vials stay in a clean preparation area. Contrast: Assume the preservative eliminates every viral risk and permits access with previously used injection equipment. This alternative fails because preservative does not provide complete protection against contamination.

Clarifying an ambiguous medication direction

A medication discrepancy is resolved through the prescribing and pharmacy process before administration. Independently choosing an interpretation is not an appropriate substitute for a clear authorized order. Worked case: A prescribed medication amount has an unclear unit. A justified response is: Clarify the ambiguous order through the prescribing or pharmacy process before administering the affected medication. Contrast: Guess the intended medication amount from a familiar drug name rather than resolving the specific order discrepancy. This alternative fails because familiarity does not resolve a specific order ambiguity.

Contrast the alternative and transfer

Before revealing an answer, explain why the tempting alternative does not fit the stated cue. Avoid overgeneralizing a rule into a slogan: the same intervention can be appropriate in one assessed situation and inappropriate in another. In the linked variants, compare the actual change, decision-making capacity, measurement or uncertainty. State which detail controls the next action rather than looking for a particular answer letter or the longest option.

Evaluate the response and preserve the limits

After any intervention, evaluate the relevant findings against the intended outcome. Report unresolved concerns and communicate who will follow up. These foundation tasks use simple single-select or numeric interactions. They are not a full clinical case simulation, official partial-credit scoring, computer-adaptive testing or a prediction of licensure success. Clinical educator review and real learner calibration remain separate from the rule-consistency checks.

WORKED EXAMPLE

A medication vial has been entered with a used syringe. Which nursing response best fits the stated findings and professional responsibility?

  1. Identify the current cue and relevant uncertainty: A medication vial has been entered with a used syringe.
  2. A medication container with questionable sterility is not used. Follow the relevant pharmacy and infection-prevention procedure rather than trying to visually certify sterility.
  3. Select the response that addresses that cue without an unsupported assumption: Discard the compromised vial using the required procedure and obtain an appropriate uncontaminated medication supply.
Answer: Discard the compromised vial using the required procedure and obtain an appropriate uncontaminated medication supply.
Compare the other answer choices
Use the contents because the solution is still clear and no contamination can be seen through the vial.

Appearance does not establish sterility.

Clean the outside of the vial and assume the known contamination of its contents has thereby been reversed.

External cleaning does not reverse contaminated contents.

Save the compromised supply for a less vulnerable patient rather than obtaining medication with verified appropriate sterility.

Compromised medication is not made safe by choosing another patient.

Try it, then explain it.

Complete a short set without looking at the example. After each answer, explain which evidence or rule supports your decision and why the alternatives do not fit.
